I would check out the UK government website he may be able to apply for the GHIC card.

Apply on line UK.Gov. Spain does not give them to UK nationals. Obtained mine couple of months ago.

Just a correction, Spain DOES give to UK Nationals if resident here and paying autónomo
With regards the GHIC, the following should be borne in mind:
What health cover is provided by the GHIC/EHIC?
Both cards give you access to medically necessary state provided healthcare while staying temporarily in an EU country, including things like:
- emergency treatment and visits to A&E
- Treatment for a long term or pre-existing medical condition
- routine maternity care, unless you’re going abroad to give birth
Please note the highlighted points. The GHIC / EHIC is not a substitute for health cover - it provides only that treatment that is medically necessary, also, more important it is only for use if you are temporarily in an EU country i.e. a tourist - it cannot be used if you are a resident of Spain to provide health cover in Spain.

Do you pay autónomo? If not, no, you can’t get Spanish EHIC, if you do, then you are entitled but must show you are traveling or intending to travel as that is its function.
With regards UK GHIC, yes, if you apply and tell them you are normally resident in UK or resident in EU and have registered S1, you can get a card. If you are a permanent resident of Spain, i.e. Spain is your country of habitual residence, you cannot use it here. If you use it here and it’s later found out you are resident, you may become liable for the cost of treatment. A GHIC cannot be used to provide full healthcare by a resident of Spain in Spain.
The only exception is if you also are in receipt of health benefits by means of S1 form.

Helpful thanks. Son is in fact autónomo now have gone self employed in September. However he has been entitled to Spanish healthcare for the last 8 years having been in employment. Is it only autónomos who are entitled to the Spanish EHIC or should it in fact cover all those for whom social security payments are made? Would seem slightly counter-intuitive if it is only autónomos

Apropos of this, I decided to try applying for a GHIC. I supplied country of residence and answered “No” to having a registered S1 and “No” to whether I was studying and was declined. This may be a bit long, but here are the requirements for a GHIC straight from the NHS website:-
Based on the information you have given us, we cannot confirm your eligibility
You may be eligible for a UK GHIC if you meet one of the following criteria:
- You are legally living in the UK and you don’t have healthcare cover provided by an EU country, Switzerland, Norway, Iceland, or Liechtenstein
- You are living in the EU with a registered S1, E121, E106 or E109 form issued by the UK
- You are living in the EU with an A1 document which is issued by the UK
- You are a family member or dependant of an entitled individual already listed.
You may be eligible for a new UK EHIC if you meet one of the following criteria:
- You are living in the EU, Switzerland, Norway, Iceland, or Liechtenstein, since before 1 January 2021 with a registered S1, E121, E106 or E109 form issued by the UK
- You are living in the EU, Switzerland, Norway, Iceland, or Liechtenstein, since before 1 January 2021 with an A1 issued by the UK
- You are a UK resident who is studying in the EU, Switzerland, Norway, Iceland, or Liechtenstein, and have been studying there since before 1 January 2021

Definitely all those for who Social Security payments (Autónomo) is paid
“Autónomo” is one of those words that has dual meanings. It refers to the Social Security payments themselves, AND to those who are Self-employed … so an Autónomo pays … Autónomo!!
