If he is a UK citizen with preserved rights under the Brexit treaty he can get a UK Ehiv

Which cannot be used in Spain if you are resident.

I think as a Spanish resident, he would have to get the Spanish EHIC; I’m not certain that a person who is not habitually resident in the UK can get the GHIC as they would not qualify for treatment on NHS etc.

If you have an EHIC as I do (resident here in Spain but retired and in receipt of U.K. state pension from pre-Brexit) that covers me for full health care in Spain and for emergency treatment in any EU country.

But.

It still does not cover for more than that which is why I have travel cover too. If I was to have a serious accident (by car or a serious health issue) in another EU country or in Britain I would need an air ambulance to get home and get local care, possibly in a hospital. The private cost of chartering this can be €25,000 or more.

Don’t take the risk. Get annual travel cover.

I do exactly as you do and always have travel insurance.  But then I have the cash to do it.  I am aware son would find it difficult to raise this for a short visit to other relatives in Portugal. So while this is a sensible move, I suspect it’s not going to work for him, which is exactly why I raised the point in the way I did.

Only if he is of pensionable age with S1, otherwise no they are not entitled to a UK EHIC.

Out of interest I looked at what I would have to pay for travel insurance as a Spanish resident, compared with the cost as a U.K. resident.  The difference is quite staggering. The multi-trip policy I found in Spain cost 236€ compared with £56 in the U.K. which also gave health cover of £5m!  Anyone know of well-priced policies in Spain?

You need to look again. We use StaySure in Spain. €148 inc existing health conditions. Nothing like the figure you suggest. Of course you are certainly free to risk paying tens of thousands to recover your body home, by avoiding any travel insurance.

Thanks for this.  At the risk of repeating myself, I always have travel insurance  - I was just surprised at the large difference in cost.  I have now checked out Staysure as you recommended and although it is much cheaper than the other Spanish policies, it is still double the U.K. price
