I believe I read something about it costing around 57€ per month but then for under 65 it then jumps to157€ quite a leap but luckly the person looking for it is under 65. 

As they are an Autónomo they are covered by the National health service but would like to augement their coverage.

If they are Autónomo, they are already covered.  Convenio Especial is only for people without any health cover needing health cover - they can’t augment what they already have, as Autónomo, the person already has the top health cover available

Ahh now it makes sense so the convenio especial is for those not paying into the Social Security coffers.

A much better answer. 

But I have a still have a  question.

What we were wondering about was while the health system in Spain is considered one of the best there are still some delays when trying to see a especialista medico. 

Now I am aware that these delays are possilby counted in weeks or months not years, looking at you HSE, what I am looking for is -can a autónomo take out a private policy to overcome those those delays to sublement what they already have?
