We also have private health care but have twice had to use Spanish emergency services because of the distance to ASSSAs private hospital. I received excellent emergency treatment at Urgencias Mojacar and was not charged. My partner was treated at the hospital in Albox. They said he would receive a bill but that was months ago and still waiting for it.
We too have health cover with ASSSA and have had to go to HO for treatment for excruciating abdominal pain on 4 occasions now. ASSSA cover emergency treatment at their listed hospitals which as previously mentioned are over an hours drive away from Albox. We have paid for all of the visits to HO, some EUR350 or so The treatment was thorough though not very compassionate and we have not got to the bottom of the underlying problem yet. If the problem was life threatening the health cover would include initial treatment wherever and then an onward transfer to a listed hospital as soon as possible. Our application for residency was rejected due to a complete and utter foul up by our lawyers, so partly due to the hospital scenario we have decided not to pursue it and retain our house for holidays only.

Unfortunately I have. They did treat me, but I got a bill a week later!

Further to my original post:
It seems that charges will be incurred for accessing public health services if you have private health, which is a requirement if you are not of pensionable age and wish to be resident in Spain.
