If a Spanish Company fails to supply the above which I have been told is the complaints procedure and they legally have to supply it, which authority in Albox do I report it to.

Your first stop will be the Ayuntamiento. They may direct you to advise Junta de Andalucia directly but the Ayuntamiento should be consulted first.

I am just going through this process against "METALBOX". I went to the local police who sent me back to METALBOX to request the form again and then call them and tell them If Ihad been refused.

When the the police arrived and were also refused, so they did an “Informe”,which I believe means METALBOX will be prosecuted?
