I have a Adjunta AN number and stamped certified letter to confirm I have had the Jab. BUT it has my Date of Birth Wrong. Can anyone help please.
You will need to go to your local medical centre to sort it out....

Send a message to the address listed on the vaccine notices. Put on your details including name, d o b and nie number and they will get back to you. Good luck.
