Non Lucrative Visa renewal after the initial 1st year

Hi,
Has anyone recently renewed their TIE card (NLV scheme) after their first year in Spain.
Do you have to show a further 2 years fully paid up Health insurance or is a Direct Debit mandate suffice for renewal?
Please, I am fully aware of the 400% IPREM and we fully meet the requirements on all aspects of the renewal process just confused on the Health.
